Your school was one of 17 schools throughout Michigan to receive a grant from Blue Cross Blue Shield of Michigan to implement a special program to improve healthy eating and physical activity among the children. My colleagues and I are conducting a program evaluation study of the programs at all 17 schools and we need your help.
First, we would like you to complete two “teacher” surveys this school year—one at the start of the school year and one at the end of the school year. The surveys have questions about your teaching practices related to the special healthy eating and physical activity program. Enclosed is an envelope labeled “Teacher Survey.” This envelope has a copy of the survey and the consent form that provides a more complete explanation of the teacher survey study. If you agree to complete the survey, please sign the consent form, complete the survey, and place them back in the envelope.
Second, we would like you to help the students in your classroom complete two confidential surveys of their knowledge, beliefs, and behaviors related to healthy eating and physical activity. The “student” surveys will also be completed early in the school year and again at the end of the school year. We would like you to read the surveys aloud and assist the children as they complete each survey. Completing the surveys will take about 40-45 minutes and it should be completed without interruption. Please follow the enclosed protocol for administering the survey.
Third, we would like you to mail your survey and the students’ surveys in the enclosed addressed, stamped envelope back to us.
Finally, we would also like to show our appreciation for you efforts by mailing you a $25 VISA gift card that you can spend anywhere you like. Enclosed is a "subject fee payment form" that you must complete and sign before we can send you the VISA gift card. Please include this form in the mailing. After we receive your mailing, we will send you the gift card.
